I think the image is showing an enlarged image of a child being painted on a wall or poster to make a statement. I think it could be both because it could also be an illusion. If the image is an illusion then I do think it is photoshopped but if it is a photograph of a man painting on a wall or poster then no. I think the intent was to show that a persons eyes can show almost all of their emotion. You can tell the girl is sad or scared based on her one eye that is shown. The scale of the image shows that it is a large image that took a lot of time to create.
